What happens if a child is named as beneficiary?
Insurance proceeds cannot be paid (directly) to a minor. If the named beneficiary is a minor, the proceeds can be placed on deposit with us until age of majority is reached or a financial guardian can be established for the minor's estate. Estates must be established by utilizing an attorney or local court systems.
If my beneficiary precedes me in death, how will my claim be paid?
If your beneficiary precedes you in death, proceeds will be paid to your contingent beneficiary. If you have not named a contingent beneficiary, proceeds will be payable to your estate.
How can I change the benefits on my policy?
To increase your policy benefits, please contact Trustmark Life Insurance Company of New York's Customer Service Department.
To decrease your policy benefits, the owner should contact Trustmark Life Insurance Company of New York's Customer Service Department to discuss the requirements. Some requests can be processed with the phone call alone but others may require a written request from the policy owner on the Policy Owner Service Request Form.
All benefit changes are done by an endorsement that reflects the requested change in coverage. If for any reason you decide not to accept the change in coverage, you must notify us within 30 days after receipt of the endorsement. Am I eligible for a partial surrender or loan from my Universal Life Insurance Policy?
There must be accumulated cash value in a Universal Life plan before any partial surrender or loan is available. An owner may then withdraw funds not to exceed that value.
Please refer to your policy to see how your policy will be affected before taking a partial surrender or a loan.
To request a partial surrender or loan, complete the Policy Owner Service Request Form and submit your request to Trustmark Life Insurance Company of New York. The request should include the policy or social security number, the request for loan or partial surrender, and the signature of the policy owner. It may be mailed or faxed to Trustmark Life Insurance Company of New York's Customer Service Department. If my insurance policy lapses, do I have the option of reinstating the policy?
As long as the initial premium payment was received, a person may apply for reinstatement. The reinstatement period depends on the type of coverage. Specific details can be found in your policy. If you wish to apply for reinstatement, please call or email Trustmark Life Insurance Company of New York's Customer Service Department and ask for a reinstatement application.
Once the application is completed, send it to Trustmark Life Insurance Company of New York. The underwriting area will review the application and the applicant will be notified when the underwriting process is concluded.
If the policy is approved for reinstatement, a letter will be sent notifying you of the premium amount due, and when the premium is received, the policy will be reinstated.
If I wish to cancel my policy, what should I do?
The policy owner should contact Trustmark Life Insurance Company of New York's Customer Service Department. Generally the request can be taken over the phone. The policy owner must be able to verify the last four digits of their social security number, the correct address, their date of birth and name(s) of persons insured by the policy.
Some employers, agencies and third party administrators require written requests in order to stop the payroll deductions and cancel coverage. In these situations it is requested that the policy owner write a letter and include their policy and/or social security number, a statement of intent to cancel and the policy owner's signature. The request can be sent to Trustmark Life Insurance Company of New York's Customer Service Department.
If I am put on leave or incur a temporary absence from my job, what should I do about my premium payments?
The policy owner should contact Trustmark Life Insurance Company of New York's Customer Service Department. The Trustmark Life Insurance Company of New York representative can help determine the best method for paying the policy premiums.
What are the options available to me if I wish to pay my premiums directly to Trustmark Life Insurance Company of New York?
There are three options available for paying premiums directly to Trustmark Life Insurance Company of New York.
1. Direct bill: monthly, quarterly, semi-annually or annual statements sent directly to your home. Payments may be made with a personal check or money order made out to Trustmark Life Insurance Company of New York.
2. Automatic bank draft: insurance premiums can be automatically deducted from a checking or savings account. The same options of monthly, quarterly, semi-annual or annually are available. Automatic Bank Draft Form
3. Credit card: payments can be made with a credit or debit Visa or MasterCard.

How can I change my beneficiary?
Complete the Beneficiary Change Form and mail or fax it to Trustmark Life Insurance Company of New York's Customer Service Department.
If I leave my employer, is my voluntary benefit insurance automatically cancelled?
No. As long as the first premium was payroll deducted, the coverage may be continued. To retain coverage, the policy owner should contact Trustmark Life Insurance Company of New York's Customer Service Department, and notify us of the change in job status. The premiums can then be billed directly to the individual or taken through an automatic bank withdrawal.
